Leave of Absence/Withdrawal Application in 2021 Spring semester has been adjusted to be online application due to epidemic prevention from May 20 to June 11.

Step1: Download Application for Leave of Absence/Withdrawal and fill out the application form

Step2: Get advisor's approval: the application form with advisor's signature, confirmation email with your advisor, screenshot of instant messenger with advisor's approval and so on.

Step3: Email IPHD office the application form.
